l Testing Hours and Check-in time:

Ø Entried into Taiwan ovrer 30 days:

1. Rapid Test (Dr. CHEN JIANN HWA): Monday to Friday 10:00 AM( 22 person per day)

2. Regular Test (Dr. CHEN WEI LUNG): Monday to Thursday 12:00 PM( 13 person per day)

Ø Entried into Taiwan within 30 days (Regular Test):

Monday to Thursday 2:00 PM(5 person per day), and please check-in before 1:50 PM. Appointments will be cancelled after 1:50 PM.

l Registration

Send the email attached the verification documents to covid19@cgh.org.tw, before the fourteen days of the tested day. The supervisor will reply the email for your registration number before the three days of the tested days.

l Verification Documents:

Ø Going abroad: Please bring the following documents on your scheduled day:

1. Passport ( resident certificate)

2. Photocopy of your electronic airline ticket.

3. The Application Form for Out-of-Pocket Polymerase Chain Reaction (PCR) Testing for Coronavirus Disease 2019 (COVID-19). (attachment 1 download)

4. Credit Card Authorization Form (attachment 3 download)

5. People who are traveling to Japan/ Hawaii should bring a copy of the test report required by Japan/ Hawaii.

Ø Entry Taiwan within 30 days or non-going abroad people: Please mail the items above and the to covid19@cgh.org.tw at least 3 days before your scheduled date. After confirming the documents, the staff who is in charge will reply to the result.

l Check-in Site: Please Wait at the waiting area of Outdoor Screening Station (the green tent), and the doctors will call your clinic number in order.

l Time to receive the report:

1. Rapid Test: Receive the report at 16:30~17:00 on the tested day.

2. Regular Test: People who get tested on Monday to Thursday may receive the report at the next day (14:30~17:00), and it will be postponed to the next working day in case of national holidays.

l The site to receive the report: The Medical Records Room (Counter A) in the lobby of the hospital.
