l  Testing information (the Fee, Hours and Check-in time, since the August 30th 2021):

Ø   $4,500 per person: Monday to Friday 9:10 AM ( 45 person per day)

     ＊　ATTENTION!

Please arrive at the waiting area of the Self-Pay COVID-19 Test Station (the blue tent) before the tested time, and the doctors will call your clinic number in order. Appointments will be cancelled if you were late.

l   Registration

Please send the email with application documents to covid19@cgh.org.tw (please follow the example for the email title) 3 weeks before the tested day.

The supervisor in charge will reply the email to you with the registration number three days before the tested days if your  application documents are complete.

l   Application Documents:

1.     Passport ( resident certificate)

2.     Photocopy of your electronic airline ticket.

3.     The Application Form for Out-of-Pocket Polymerase Chain Reaction (PCR) Testing for Coronavirus Disease 2019 (COVID-19). (attachment 2 download)

4.      Credit Card Authorization Form (attachment 3 download)

5.      Personal Information of self-pay COVID-19 test, Cathay General Hospital (attachment 4 download)

6.      People who are traveling to Japan/ Hawaii should bring a copy of the test report required by Japan/ Hawaii (attachment 5 download Japan/ Hawaii).

l   Time to receive the report:

1. Receive the report at 16:30~17:00 on the tested day, 9:00~17:00 on the next day. 9:00~12:00 on the Saturday. Service was not provided on the Sunday and national holidays.

2. Please bring your passport for receive your passport.
